Home Health Care
Home Health Care avoids costly institutional care by teaching family members to care for the sick and disabled at home. The program assists patients and their families in resolving the grief and loss associated with illness and death, and it collaborates with other health care professionals in assessing the communitys needs.
Medicaid funds the program. Its services include maternal and child care, social work, personal care, family counseling, pastoral counseling, health education, transportation, adult day care and placement screenings for nursing homes.
The Home Health Care staff includes Registered Nurses, Home Health Aides, Social Workers and administrative personnel. All Home Health Aides have gone through a Home Health Aide training course that includes classroom instruction and a clinical practicum. Home Health Aides hold New Jersey state certification.
Many of Home Healths Home Health Aides are community residents
